Providence's Partner CCIN Awarded Second CMS Innovation Grant

Capital Clinical Integrated Network (CCIN), a partner of Providence Hospital, has been awarded a CMS Innovation grant of $14.9 million. Providence is the only hospital in the District of Columbia to partner with the CCIN network.

This 3-year Health Care Innovation grant from the Center for Medicare and Medicaid Services (CMS) is designed to support innovative projects that will save money, deliver high quality medical care, to persons enrolled in Medicare, Medicaid and Children’s Health Insurance Program (CHIP), particularly those with the highest health care needs. Providence will work with Mary’s Center and our other partners to implement and test an integrated clinical network to target the chronically ill within the population of Medicaid patients who do not seek regular medical care from a primary physician but instead use expensive services such as emergency room visits.

The project will use a city-wide database, care teams, and tele-health to communicate with patients, develop care plans for them, and personally manage their care as they are gradually transitioned into patient-centered medical homes. Over a three-year period, Mary’s Center for Maternal and Child Care will train and hire 44 health care workers to serve as care managers and community-based care coordinators. Partners in the CCIN include: Chartered Health Plan, Cisco, CoreBTS, DC Behavioral Health Association, DC Primary Care Association, Green Door, La Clínica del Pueblo, Mary’s Center, Providence Hospital, So Others May Eat, United Healthcare, and Unity Health Care.

The total amount of the three-year grant is $14,991,005, and the projected savings to the community are $17,712,000, resulting from reduction in duplicative services and, fewer hospitalizations and lower transportation costs. Over the course of three years 10,800 patients will participate in the program and 51.5 new jobs will be created.
